Required Reading:

• At least one week prior to surgery, read the instructions before surgery.

• Read your surgery consent form before your day of surgery: Removal of foreign body in the airway or upper digestive tract (anywhere in the nose, mouth, throat, esophagus, or lungs) consent (form “C-AW”). Notify your surgeon if you have questions about the risks of surgery or the consent form.

• Adults should complete their Advance Directive forms.
